Tim writes in to defend RFK's "SSRIs harder to quit than heroin" line. First-person experience: SSRIs are not addictive in the heroin sense, but withdrawal triggers a 1-month "incredibly crazy bipolar mood swing" period most people (and most doctors) don't know about.

Dear Adam,

Now I'm writing in to comments about SSRI's because you're playing more news clips about people talking about RFK talking about them.

I took SSRIs for several years from when I was about 16 to when I was about 23. I have very strong and fairly negative opinions about them, and I think you and I are in strong agreement on just about everything. I would like to point out one thing that I have found is not widely known.

In one of the clips played on the show, a newscaster is attempting to make RFK look ridiculous by saying he said SSRIs are harder to quit than heroin. I have no idea if he said that or not, but I want to tell you that that is not necessarily a ridiculous thing for him to have possibly said.

SSRIs are definitely not harder to quit than heroin, but that is only a commentary on heroin, not on SSRIs. SSRIs are incredibly difficult to quit, and most people are not aware of this. In fact, my doctor was not aware of this, and that is how I found out the hard way.

The long and short of it is that you go incredibly crazy in a sort of bipolar mood swing kind of way for the duration of your withdrawal, which is about a month. Medical best practice is to very precisely titrate off of the dose over a span of several months. But, again, as I found out the hard way, a lot of doctors don't know they're supposed to do that, and don't tell you to do that.

It's not addictive in the way that heroin is. It's not like you feel like you really just need one more SSRI. And I can't speak from experience, but I'm certain that withdrawal from heroin is more severe. But you go crazy when you are quitting SSRIs, and it can be very difficult to resist the urge to take an SSRI to make yourself stop going crazy while you power through it.

And so the kernel of truth in what he's saying is that SSRIs are very difficult in practice for people to quit. And since they are not addictive in the conventional sense, most people don't realize this.

In fact, one of the biggest reasons why SSRIs are handed out so freely is because doctors do not believe that they are difficult to quit. It's seen as no big deal like, hey, we'll try this medication if it doesn't work. Oh, well, we'll try a different one.

It made me extremely agitated, not to mention aggressive. And I am normally a pretty relaxed person. While I was taking this, I started spending most of my weekends drinking black coffee watching very violent video games on YouTube. The kind of games where you're ripping monsters in half with a chainsaw. I had no interest in playing these games. I just enjoyed the graphic violence. Again, normally I am a pretty relaxed person. But these drugs made me aggressive and hypomanic.

SSRIs boost serotonin. And serotonin is a substance that makes you feel safe and dominant. So it's not hard to imagine why this could lead to dangerous behaviour. Of course, doctors are not at all aware, since these kinds of side-effects can easily be attributed to something else. Maybe you're having a difficult time at work and you just need to meditate. And doctors mean well, but don't have the time. So maybe they just prescribe you some benzos to calm you down. At any rate, getting off SSRIs is its own form of torture, with mood changes, brain zaps, and lots of other fun stuff that will haunt you until the end of your days.

Getting off these drugs was absolutely the right thing for me. And I think listening to the show first put into my head how dangerous these drugs can be.
